Mason Elite dentistry membership plan
With the bulk of our practice being made up of patients with no insurance or insurances not in-network, we wanted to offer another option. The Mason Elite Dentistry Membership plan is a yearly plan that can begin on any date. There is no waiting period, deductibles or pre-authorizations needed for treatment. The plan will cover two preventative cleanings, two periodic exams, any necessary radiographs and two fluoride treatments if indicated. The plan will also cover two emergency exams per year.
Patients participating in the membership plan will receive 15% off any necessary procedure not covered under preventative services (see exclusions below). Periodontal disease services will be offered at 50% off including periodontal maintenance and scaling root planing treatment.
Our membership plan is not insurance, a qualified health plan under the ACA, a prepayment for future services or payment for access to discounted services. Rather, our membership is a direct payment arrangement for services, products and care provided.
Our membership plan is not combined with insurance or other offers. Our plan does not cover services completed at a specialty office or any other office. Our plan does not cover lab related expenses or procedures such as dentures, partial dentures, cosmetic veneers or cosmetic veneer wax-ups.
Plan Exclusions
Any service that is performed outside of our office is not covered. It is not a discount for any specialist visits. It will not apply to any work that requires outside lab assistance (some implant crowns, dentures, partial dentures, flippers, cosmetic dentistry). Cosmetic dentistry is not covered including veneers and veneer wax-ups.

In-Network Insurances
We are in-network with Dental Care Plus, Medical Mutual, Sunlife and Superior insurance. We are able to bill other insurance plans. We accept full payment up-front for out of network carriers and the insurance company will reimburse you directly if your claim is approved. There is no guarantee that your plan will pay. Medicaid and state insurances typically do not accept out of network claims. If a in-network dentist is your top priority, the insurance plan website is the best resource to find dentists.
